DOG ADOPTION APPLICATION

Please take the time to fill out ALL of the questions.

For a printable version of this application, please click here

PLEASE READ BEFORE FILLING OUT THE APPLICATION

NOTE: We do not do same day adoptions.  After submitting your application, it will be reviewed by our adoption committee, we will then contact you to discuss your application.  If the application is approved, a home visit is required for all dog adoptions.  We appreciate your interest in helping a rescued dog in need.  Carolina Animal Protection Society of North Carolina is an all-volunteer organization, and our foster dogs reside in private volunteer homes throughout Onslow County.  There is no facility or location where all the dogs can be visited.  We receive a tremendous number of email inquiries and applications weekly.  If you do not hear from us within 2 weeks of submitting your application, it is because we do not currently have the right dog for your environment.  We will hold your application on file and may contact you in the future should we take in a rescue dog that is appropriate for your home.
